This Interactive Monster Maker Visual Motor Activity “Escape Room” is perfect for your teletherapy session with individuals working on visual motor skills, following directions, and gross motor movements! Make a series of choices to create and draw your own monster. When you’re finished, complete a few gross motor movements to decide how your monster moves!
Using Google Forms, Kerri Healey, COTA has created this fun and engaging visual motor and gross motor activity.
The PDF with the directions to make a copy of the Google Form for your drive will be available electronically immediately following payment. You will need a Google Drive and internet access to use the resource.
How Does the Interactive Monster Maker Visual Motor Activity Work?
- Download the PDF to make a copy of the Google Form.
- Click on SEND to get a link to complete the Monster Maker activity.
- Students can make their monster choices, draw and color their monster on paper then complete exercises to complete the monster maker activity!

How to Use the Google Form
When you open up your copy of the Google Form it will be in QUESTIONS mode or edit mode. You need to get the link to give access to the student to make the choices and progress through the activity. Click on the SEND button to get the link. Copy the link and send it to your student or open it up on your computer if you are sharing your screen or in person. Now the student can complete the activity.
Once the student hits SUBMIT the response will be saved and you can view what activities they completed in the Responses section. If you want to collect data on individual students you will need to make copies of the form for each student.
Each purchase is a single license, for a single teacher, therapist or parent. If additional professionals wish to use the product, the buyer must purchase additional copies.
